This position will work in partnership to provide targeted and opportunistic chronic disease screening, health promotion, follow up and education.
Activities include facilitating smoking cessation, assisting with lifestyle programs, data collection, adult health checks, linking Aboriginal community members with mainstream services, and engaging with local Aboriginal communities to meet local health needs.
This position will work closely with Awabakal Aboriginal Medical Service as well as mainstream services under a partnership arrangement for the communities within the Greater Newcastle Cluster which includes Karuah, Toronto, Belmont, Morisset, Windale, Raymond Terrace, Waratah, New Lambton, Newcastle, Tea Gardens/Hawk’s Nest and Nelson Bay.
This position will assist in the intervention, prevention and referral of Aboriginal clients, families and carers to Clinical Nurse Consultants, Nurse Practitioner and General Practitioners for assessment, case review and development of clinical management plans for those who are identified as being at risk of developing or with established chronic disease, to access health service.
This is a designated Aboriginal/Torres Strait Islander Position. Applicants for this position must be of Aboriginal descent through parentage, identification as being Aboriginal and being accepted in the community as such.
