This position will work as part of the Oncology Team at Wagga Wagga working across the various services including WWBH, Riverina Cancer Care Centre, Community Health, with GPs, Medical Specialists and other cancer service providers to increase access and ensure the effective coordination of care to cancer patients. The treatment pathway for a person diagnosed with cancer is often complex. In many cases the quality of treatment and the quality of life for the patient may be improved through the assistance of a Nurse Cancer Care Coordinator. The role will develop a model of care coordination that will assist patients accessing treatment across cancer services
The position is also required to provide regular out-reach to Temora, West Wyalong, Cootamundra and the surrounding region, consistent with Greater Southern Area Health Service priorities. Establishing clear referral pathways with local GPs and service providers will ensure patients with a diagnosis of cancer and their families/carers are appropriately referred.
