We provide support to people living in there own home in the Inner West, South West and South East Sydney. Your role will be to work at the individual, family and community level to support each person to determine their own needs and participate in the community. This support consists of a mix of support coordination, in home and community support.
Applicants will require:
- Experience working with people with a disability
- Experience in developing and implementing individual service plans and coordinating support to individual clients
- Experience in liaising with service providers in the community and disability field
- Current drivers license and comprehensively insured vehicle
