The Client
Our client is a front runner in the provision of exceptional support services to the local Community. They provide community care services for the aged, people with disabilities and their carers. They are currently going through an exciting period of growth.
The Position
We are looking for 2 Case Manager’s to join their Compacks team. The Compacks program provides people with a case managed package of care for up to 6 weeks after discharge from hospital. The concept is to case manage community support jointly with a multidisciplinary team, starting before discharge and continuing for a short time after discharge, with the aim of facilitating access to mainstream community services.
The Ideal candidate
The ideal candidate will have Tertiary qualifications in the human services field and/or a minimum of 3 years experience in a similar role. You must have demonstrated experience working with people with a range of complex needs and have an in depth understanding of the principles of case management.
You should be a compassionate individual, with strong communication and excellent organisational skills, with the ability to prioritise your work and work autonomously. You will have an enthusiasm for the community services industry and a genuine desire to help others and improve the community. Knowledge of the HACC service system will be highly regarded.
