.  undertake in-home assessments of clients with a diverse range of needs

.  identify, co-ordinate & monitor appropriate services for eligible clients and to produce appropriate care plans

 Essential Selection Criteria:

.  clear committment to health & welfare issues

.  a degree in social work, nursing, allied health or a related field

.  a sound understanding of service & funding guidelines, policies & procedures relating to the aged & disability, primary health & community support sectors

.  the ability to work compassionately with marginalised groups in a complex & sensitive & culturally diverse service area

.  excellent oral & written communication skills

.  a current Police Check Certificate , First Aid Certificate, and are in good health
