We are on the lookout for an experienced Intake Officer or Case Management professional to join this growing organisation in Sydney's Western suburbs.

If you have experience in the Aged Care, Mental Health or Complex Needs areas then we would like to hear from you.

This role essentially will look after the initial screening and eligibility process for a range of services and options available to the target client group. You will work very closely with other team members, and need a comprehensive knowledge of community based services available. 

To be successful in securing this exciting part time opportunity you will need to possess the following skills and experience:

  • 2-3 Years experience in Community and Welfare
  • Current Drivers Licence
  • Prior Case Management experience
  • Experience with Aged Care or HACC highly regarded
