Project Manager
· Facilitate the development and delivery of Regional Chronic Disease Management Services for the Severe Chronic Disease Management Program as per the Caring ????? ????? which will enrol and case manage patients with severe chronic disease who frequently present to hospitals and Emergency Departments (EDs). · Work with Area Health Services, GP stakeholders, NGOs, local government, and other agencies to build collaborative and connected services reflecting the directions of the National Hospital and Health Reform Commission report · Undertake State-wide project planning to implement services across NSW · Undertake audits and mapping of existing chronic care/disease management services to ensure all services are identified and connected, and new funding builds on this · Develop and facilitate formal agreements at regional and State level to build connected and collaborative services across NSW · Develop KPIs, undertake performance analysis and oversight evaluation of the new service delivery · Assist in developing program practices and guidelines, procedures and policies for Area health Service, regional providers and co-sourced providers Coordinate and prepare progress reports etc
