Grade 1 / 2 Pharmacist (Medication Reconciliation)

We are dedicated to providing specialist care for eye, ear, nose and throat conditions through our clinics, emergency services and expert surgery.
An opportunity exists for a Grade 1 or 2 Pharmacist (Medication Reconciliation). This position will offer you the opportunity to develop clinical, organisational and people skills in an expanding and friendly environment. You will make a difference to pharmacy and hospital practice by developing and facilitating the role of medication reconciliation (on admission) to acheive continuity in medication management.
The position is rotational with exposure to all aspects of the pharmacy service, including medication reconciliation on admission, clinical ward services, outpatient dispensary, sterile manufacture, research, training and education. The position is also responsible for developing and overseeing the role of medication reconciliation (on admission).
To succeed in this role you must be a registered pharmacist in Australia with clinical experience in hospital pharmacy. A commitment to continuity in medication management, medication safety, quality improvement, education and research is essential. Familiarity with the PBS, Merlin dispensing software and RiskMan would be highly regarded.
