. 48 hours per fortnight - 3 days per week
. Based in West Geelong

We are currently seeking an experienced and enthusiastic community clinician for this permanent part time position. The position is open to Registered Nurses and Allied Health professionals. Experience working with clients with chronic disease and/or psychosocial needs in the community an advantage.

The community clinician is an integral member of the HARP multi-disciplinary team, providing assertive case management for HARP clients. The key objectives of this position are to improve patient outcomes by redirecting clients away from unnecessary acute services and reducing length of stay, providing client centred care based on needs identification, linking into more appropriate community services, and advocating where needed.
