An exciting opportunity exists for a Division 2 nurse to become part of the team at Prague House - a specialist aged care facility that cares for both men and women with a mental health issue or an alcohol related brain injury, who have a previous history of homelessness.
As Care Coordinator you would be responsible for communicating with G.P's, Specialist Clinics, and families. An ability to work closely with all members of the team and to have the best interests of the resident at heart are essential in this position.
Experience in Residential Aged Care would be advantageous.
