Full Time - Maternity Leave Replacement
This is a rare opportunity for a leader with relevant experience in acute and/or community health to manage an exciting and innovative team. ALERT is a ‘virtual’ multidisciplinary team with staff located at St. Vincent’s Hospital and local Community Agencies to achieve improved client outcomes.
ALERT aims to enhances discharge planning and continuity of care for patients’ who are (or are at risk of) presenting to the ED due to their complex medical and social situation. ALERT offers co-ordinated care for patients including advocacy for treatment options and service linkages, along with risk screening in the ED, assessment of the patients’ bio-psychosocial risk factors and short term assertive outreach for patients identified at risk. The key target groups are patients with complex needs resulting from; substance use, mental health, homelessness, disability or complex aged care issues.
