The Slow Stream Rehabilitation Coordinator is responsible for providing therapeutic services, which meet the individual and group needs of aged clients accessing the Slow Stream Rehabilitation Program / Day Care Centre. The role integrates the ongoing assessment and management of clients with functional and cognitive impairment, development of care plans, liaison with Health Professionals and carers.
