We are looking for an experienced and enthusiastic Diabetes Nurse Educator who enjoys working in a multidisciplinary team. The role is challenging and diverse and includes work in Diabetes Direct our HARP (Hospital Admission Risk Program) and building our community health diabetes program.
The role includes, providing care coordination, assessment, treatment and education to patients with diabetes. A strong interest in self-management approaches and group work skills will be highly regarded.

Salary is based on the Nurses Award Grade 4B
