Adolescent Community Placement – Team Leader

The Adolescent Community Placement Program Team Leader will directly supervise a team of staff providing case management and community placement support services to young people living in home based care in the Loddon Campaspe region. The ACP team provides emergency, short and long term placements for 13-18 year olds residing with St Luke’s volunteer caregivers.
